"I give and bequeath to the Houston Library Board, federal tax identification number 74-1926301, 500 McKinney, Houston, TX 77002, the sum of $ ___________ to be used for its general purposes or ______________ (indicate purpose, i.e. books and materials, programs, technology, etc.)."
